Community Program Services

Community Integration Services (Social Rec.)

The Community Intergration program provides services for children, adults, and seniors. Community Intergration services are designed to be a community integration program in a 1:1. This is a time limited service designed to assist consumers with developing social skills, mobility training, safety skills, community awareness, and appropriate behavior in the community setting. This service is a time limited service to be faded when the period of time in program has been achieved. The length and amount hours of services will be determined by individual’s CSC. Typical activities could include interaction at the YMCA/YWCA programs, school activities, Community Recreational Centers and other activities to be determined by consumer choice.

Personal Assistant

Personal assistant program provides life-enriching services which promote choices, quality of life, respect and dignity for all adults and children with challenges. This program is to work on goals identified in the IPP, such as basic living skills, community participation, independent living skills and pre-vocational skills.

Community Activity Support Service

Community Activity Support Service is a program that provides life-enriching services which promote choices, quality of life, respect and dignity for adults with challenges. This program enhances the individual’s independence in the community and can be a support to assist individuals acclimate to college environments by providing one-on-one support. This program is person-center and based on participates IPP goals.
